Great Teacher Onizuka was adapted for live action twice (1998 and 2012). In 2012 An 11-episode live-action Japanese television drama adaption directed by Imai Kazuhisa was broadcast from July 3, 2012 to September 11, 2012. The series starred Akira (Ryohei Kurosawa) as Onizuka. An Autumn special was broadcast on October 2, 2012, followed by a New Year's special on January 2, 2013, and a Spring special on April 2, 2013. A second season aired in 2014.

At a bus stop, Miyabi talks a timid Fuyutsuki into asking some delinquents to leave. They harass her, but Onizuka shows up and scares them away. At the school, Anko and her friends drag Noboru outside and extort money from him, shoving him in a trash can when he can't pay. Onizuka is working part-time as a gardener at the school. He sees Noboru about to jump off the roof and taunts him out of it. In the cafeteria, a group of delinquents break a window and start causing chaos. Uchiyamada demands Onizuka do something about it, but Onizuka suplexes him for calling the delinquents "scum".
Noboru follows Onizuka to Ryuji's Cafe Nagisa, where Saejima apprehends him. Noboru explains his troubles, and Onizuka helps him get back at Anko and her friends by taking embarrassing pictures of them. Noboru still blames himself for failing to stand up for Anko years ago when she was bullied. Anko meets with Onizuka that evening and thanks him for "setting her straight", and asks to go home with him. Fuyutsuki sees the two of them walk off together. At the place Onizuka shares with Ryuji and Saejima, Anko starts taking off her clothes, and her yakuza accomplices take a compromising photo of Onizuka. Onizuka's friends come back and help him beat the guys up. The yakuza demand Anko pay them with her body, and drag her to a warehouse. Noboru calls for help, but Onizuka tells him to deal with it on his own. He tries to save Anko, but is no match for the thugs. Onizuka drives his motorcycle through a window and beats up all the guys, saving the students.
Anko explains how her parents have become distant, and Onizuka comes to their house with a sledgehammer, where he breaks down a wall. Noboru punches into the hole, symbolizing his reconnection with Anko. The next day, Director Sakurai of Meishu Gakuen School (who was the cafeteria lady from earlier) finds Onizuka and offers him a job there as a teacher. The other teachers make him worry that the class will be trouble, but they seem fine to him. Miyabi asks Onizuka to bring her friend Miki back to school. She then calls Miki, who is alone in an house surrounded by shredded furniture, and tells her that Onizuka is on his way.

Differences from the manga[]
- Onizuka doesn't want to become a teacher from the beginning, and in the show it's Mrs. Sakurai who suggests it.
- The Musashino Public High School arc with Nanako Mizuki is cut, and her story is given to Anko.
- Anko and Noboru were childhood friends in the show.
- In the show, Miki Katsuragi, Seiya Dojima, and Haruka Kuwae are students at the school in Tokyo, rather than living in Shonan.

- Azusa's actress, Miori Takimoto, is less than a year older than some of the actors playing her students, such as Tsubasa Honda, and is actually younger than the actor playing Fujiyoshi, Yuki Yamada.
